Notice of Patient Protections Against Surprise Billing

Your Rights and Protections Against Surprise Medical Bills
Federal law provides protections against certain unexpected (“surprise”) medical bills. These protections apply in limited situations, primarily involving emergency care or services provided in hospital or facility-based settings. This practice provides outpatient mental health therapy services and case management, in a private office or via telehealth. Services are scheduled in advance and are not provided in emergency, hospital, or facility-based settings.
What Is Balance Billing (Sometimes Called Surprise Billing)?
When you receive health care services, you may be responsible for out-of-pocket costs such as copayments, coinsurance, or deductibles. If you receive care from a provider who is not in your health plan’s network, that provider may bill you for the difference between their charge and the amount your health plan pays. This is called balance billing. Surprise billing most often occurs when patients cannot choose their provider, such as during emergencies or in hospital-based care.
Emergency Services
This practice does not provide emergency services. If you believe you are experiencing a medical or psychiatric emergency, please call 911 or go to the nearest emergency department.

Good Faith Estimates

If you do not have insurance or choose not to use your insurance, you have the right to receive a Good Faith Estimate of the expected cost of services before they are provided. If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges.
Questions or Complaints
If you have questions about your rights under the No Surprises Act or believe you have received a bill that does not comply with federal law, you may contact the No Surprises Help Desk at 1-800-985-3059, Website: www.cms.gov/nosurprises/consumers. You may also contact the U.S. Department of Health and Human Services for additional information.
